CSD Claim Form

The City of Lethbridge administers multiple grants from the provincial and federal governments, as well as municipally tax-supported programs, through the Community Social Development (CSD) department.

Approved claimants must use the CSD Claim Form when submitting monthly, quarterly or one-time reimbursement claims.

Please note:

  • Claimants with approved agreements are eligible to submit a claim, subject to the terms and conditions outlined in their agreement
  • Claims are subject to review and audit
  • Approved claims will be paid within 30 days from receipt of required documentation
